BSA physical form

As of January 1st, 2010 ALL scouts ( tigers up to boy scouts ) AND scouters ( adult leaders ) are required to use this NEW BSA medical form.  The form is good for 1-year and it MUST BE SIGNED/COMPLETED by a doctor.

This form is ONLY good for 1-year from date of doctor’s signature
